As a courtesy to you, Hagen Dental Practice will submit treatment charges to your dental insurance carrier. Please bring a completed dental insurance form to the office at your first visit.

Please note that Hagen Dental Practice's recommendation for treatment is based on what the doctor considers best for a patient's dental health. However, this recommendation may not always be completely covered by a patient's insurance policy. You will be responsible for any fees your insurance does not pay.

All patients are charged the same fees for the same procedures, regardless of insurance coverage. Remember, your dental health is this office's first priority.
